What Most Adults Over 45 Get Wrong About Blood Sugar

An endocrinologist's perspective on what really matters for blood sugar and metabolic health as we age.

Many people are doing more right than they realize. The challenge is that blood sugar and metabolism are more complicated than simply avoiding dessert or cutting carbohydrates.

Healthy metabolic habits are built over time, not overnight.

I hear a version of the same story surprisingly often.

Someone tells me they've started eating better. They've cut back on sweets, they're trying not to snack late at night, and maybe they've even started walking after dinner. They feel like they're doing everything they're supposed to do.

And then comes the question: “Why does it still feel like my body isn't cooperating?”

It's a completely understandable question.

For years, the conversation around blood sugar has often been reduced to something that sounds very simple: eat less sugar, lose weight, exercise more.

Those habits can absolutely matter. But your metabolism is not a light switch. It's an incredibly complex system involving your muscles, liver, pancreas, hormones, sleep, stress, activity levels and the way your body responds to food.

One thing I always want people to understand is that progress doesn't require perfection. You don't need to eat a flawless diet or exercise every single day to start building healthier habits. Consistency tends to matter far more than having one “perfect” week.

“But I'm Already Eating Better.”

This is probably one of the most relatable frustrations.

You swap the sugary breakfast for something with more protein. You start drinking more water. Maybe you're taking the stairs a little more often. You make an effort to stop eating when you're full instead of finishing everything on the plate.

These changes matter.

But blood sugar regulation doesn't happen in isolation. Your body is constantly deciding where glucose should go, how much energy should be stored, how much should be used immediately, and how different tissues should respond to insulin.

Here's a situation many people recognize:

You eat reasonably well all day, but you slept five hours the night before, sat at a desk for most of the afternoon, rushed through dinner, and went to bed stressed. Nutrition matters, but those other parts of your day matter too.

That's why I prefer thinking about metabolic health as a collection of small signals rather than one single decision.

  • What you eat influences how quickly nutrients enter your bloodstream.
  • Your muscles can use glucose for energy, especially when you're active.
  • Sleep can affect appetite, energy and metabolic regulation.
  • Stress can influence hormones involved in energy balance.
  • The overall pattern of your habits usually matters more than one meal.

One Habit I Often Recommend Is Surprisingly Simple

People sometimes expect health advice to be complicated.

One of the simplest things many people can do is move a little more throughout the day.

That doesn't necessarily mean spending an hour in the gym.

It may mean taking a short walk after dinner, standing up more often during the workday, parking slightly farther away, gardening, doing household work, or simply breaking up long periods of sitting.

These habits don't look dramatic on social media, but they're the kind of things people can realistically continue for months and years. In a randomized crossover study involving 41 adults with type 2 diabetes, researchers compared one 30-minute daily walk with three 10-minute walks taken after the main meals. Overall post-meal glucose exposure was about 12% lower during the post-meal walking approach, and after the evening meal specifically it was about 22% lower (Reynolds et al., 2016). That doesn't mean everyone will experience those exact changes, but it shows why something as ordinary as when you take a walk can be worth studying.

Research Perspective
Glucose metabolism involves far more than the amount of sugar in a meal.

Scientific research continues to examine how physical activity, sleep, nutrition, body composition and cellular signaling contribute to metabolic health. No single factor operates completely on its own.

So Where Does Berberine Come Into This?

This is where the conversation becomes interesting.

Berberine is not a new compound. It is naturally present in several plants and has been studied for years in connection with different aspects of metabolism.

Researchers have been particularly interested in how berberine may interact with pathways involved in glucose regulation, cellular energy use and lipid metabolism. A 2023 systematic review and meta-analysis brought together 20 randomized controlled trials involving 1,761 participants. Compared with placebo, berberine was associated with an average reduction in fasting glucose of 0.52 mmol/L. The analysis also reported an average 0.41 percentage-point reduction in HbA1c, a 2.36 mU/L reduction in fasting insulin, and a 1.81 mmol/L reduction in two-hour post-meal glucose (Zhao et al., 2023). These pooled findings do not mean every person or every berberine product will produce the same results.

That research is one reason berberine has become so widely discussed in the wellness world.

But I think this is also where people need a little perspective.

My Perspective

I don't like presenting any supplement as a shortcut. If someone is sleeping four hours a night, barely moving, and eating poorly most of the time, a supplement doesn't suddenly erase those habits. I think it's much more useful to ask whether an ingredient can complement a routine that is already moving in a healthier direction.

I Also Think People Get Overwhelmed by Supplements

Walk into almost any supplement aisle and it's easy to understand why.

One bottle says metabolism. Another says glucose support. Another says energy. Then there's cinnamon, turmeric, herbal extracts, capsules, powders and drops.

Before long, someone who just wanted to improve their routine is standing there wondering whether they need six different bottles.

In most cases, simpler is easier to maintain.

That's part of the reason botanical ingredients such as berberine, Ceylon cinnamon, bitter melon and turmeric are often discussed together. Each has been studied independently for different biological pathways, although the strength of evidence differs between ingredients and studies.

Berberine

A plant-derived compound studied in research involving glucose and insulin-related outcomes. In a meta-analysis of 20 randomized controlled trials involving 1,761 participants, researchers reported an average 0.52 mmol/L reduction in fasting glucose and an average 0.85-point reduction in HOMA-IR (Zhao et al., 2023). These findings describe the interventions studied in those trials and are not guaranteed results from any particular commercial product.

Ceylon Cinnamon

Cinnamon has been investigated for its relationship with glucose-related metabolic markers. A 2024 systematic review and dose-response meta-analysis evaluated 24 randomized controlled trials in people with type 2 diabetes and reported statistically significant effects on fasting blood glucose, HbA1c and HOMA-IR, although the size of the effect varied across studies (Moridpour et al., 2024).

A botanical traditionally consumed as food and studied for its interactions with pathways involved in metabolism.

Turmeric

A widely researched botanical containing naturally occurring curcuminoids. A 2023 systematic review and meta-analysis included 59 randomized controlled trials and found that turmeric or curcumin supplementation was associated with an average reduction in fasting blood glucose of approximately 4.60 mg/dL and an average reduction in HbA1c of approximately 0.32 percentage points (Dehzad et al., 2023). These are pooled averages across different participants, doses and formulations.

What I Would Focus on First

If someone asked me where to begin, I wouldn't start by telling them to completely redesign their life on Monday morning.

That's usually not realistic.

I would focus on a few habits that feel manageable.

  • Build meals around foods that keep you satisfied rather than constantly hungry.
  • Include regular movement, even if it's simply walking more throughout the day.
  • Give sleep the same attention you give nutrition.
  • Try to create routines you could realistically maintain six months from now.
  • Treat supplements as support, not as the entire strategy.

There's nothing exciting about advice like this.

But that's almost the point.

Sustainable health habits often look surprisingly ordinary. A better breakfast. A twenty-minute walk. Going to bed a little earlier. Drinking water instead of another sugary drink. Repeating those decisions often enough that they eventually stop feeling like decisions.

A useful question to ask yourself:

Instead of asking, “What is the fastest thing I can do?” try asking, “What could I realistically continue doing next year?” That small change in thinking can completely change the way you approach wellness.

Where a Simple Formula Can Fit

Some people prefer using individual ingredients. Others want something simpler that fits into the routine they already have.

That's where a combined botanical formula can be convenient.

The important distinction is that convenience does not prove effectiveness and does not make a product a replacement for healthy habits. The Biggest Mistake I Think People Make

People often assume that improving their health means changing everything at once.

They clean out the pantry, start an intense workout plan, buy several supplements and promise themselves they're going to be perfect starting tomorrow.

A few weeks later, life gets busy.

That's normal.

In my view, a much better approach is building a routine that still works on the days when you're tired, busy or simply not feeling particularly motivated.

That's what sustainable health usually looks like.

The Bottom Line

Blood sugar and metabolic health are influenced by far more than one food, one supplement or one decision. Nutrition matters. Activity matters. Sleep matters. Stress matters. Research gives us useful examples: in one randomized trial, walking for 10 minutes after each main meal was associated with 12% lower overall post-meal glucose exposure compared with one 30-minute daily walk in adults with type 2 diabetes (Reynolds et al., 2016). A separate meta-analysis of 20 randomized trials involving 1,761 participants found that berberine supplementation was associated with reductions in several glucose- and insulin-related measures (Zhao et al., 2023). These findings describe particular study conditions and should not be treated as promises of individual results.

My preference is always to look at the bigger picture. Build the habits you can maintain, keep your expectations realistic, and think of supplements as one possible supporting piece rather than the entire solution.
